A dental practitioner may submit his application for annual practicing certificate using the prescribed Form 25 of the Dental Regulations 2021, pay the requisite fees and will then be issued with an Annual Practicing Certificate (APC). All information (except for no. 8 and no. 9)* MUST BE COMPLETED. Any incomplete application will not be processed. The APC must be renewed each year, and applications for APC for the following year must reach the MDC not later than 1 November of the current year.
The fees are as below:
a) One principal practicing address only – RM 100
b) Two practising addresses – RM 200
c) Three or more practising addresses – RM 300
Charges for late application of RM100 will be charged per practitioner. Other charges with regards to APC are:

1) Manual payment : Postal Order or Bank Draft made out to the “Pendaftar Pergigian”. Cash and personal cheques are not accepted. -

2) Online payment : effective 25th October 2023 via the Malaysian Healthcare Practitioners' System (MHPS)
Dental practitioners are required to have a separate APC for each premise of practice and are required to exhibit the APC at each premise (Section 42 of the Dental Act 2018).
* Late fee for 2024 APC Application will not be imposed for application received before 1st February 2024 via MHPS
*The requirement for sufficient Continuing Professional Development (CPD) points and Professional Indemnity Cover will be in force on 1st January 2025.As such, CPD points gained in 2023 are required to be submitted in the application of APC in 2024 for APC 2025.
